    1. Any material misrepresentation or deliberate omission of a fact in my application may be justification for refusal or immediate dismissal from employment.
    2. It is my understanding that EBS will make a thorough investigation of my entire work history and may verify all data given in my application, related papers, or oral interview. I authorize such investigation and the giving and receiving of any information by EBS and release from liability any person or entity giving or receiving such information. I understand that falsification of data given or discovered as a result of this investigation may result in refusal or immediate dismissal of employment.
    3. I agree that my employment may be terminated at any time without liability for any additional compensation, wages, or salary except such as may have been earned at the date of termination. If EBS management at any time requests search of my person or assigned work area, I agree to submit to such search and waive all claims for damages, if any, that may result. I authorize any physician or hospital to release to EBS any necessary information to determine my ability to perform the duties of the job I am being considered or was hired for.
    4. I understand and accept that business needs may at times require overtime, shift work, a rotating work schedule, or a work schedule other than Mon-Fri.
    5. It is expressly understood that EBS has a drug testing policy, and any employment would be conditional upon satisfactorily passing a pre-employment drug test and complying with the terms of the drug and alcohol policy. I understand and accept these pre-employment conditions.
    6. I further understand and agree this is solely an application for employment and that no employment contract is intended or being offered.
    7. I understand and agree that if I am employed, such employment is for no definite period of time and that EBS can change wages, benefits, and conditions at any time.
